Consent form for photo shoots, video shoots and case studies (aged 16+)

This form is for individuals agreeing to take part in a photo shoot, video shoot, and/or case study for UCAS.

It will only take a minute, but it is important to read this consent form carefully so you’re happy you understand its contents. We will ask you to confirm your agreement by completing your details below. Sorry, but you cannot participate until the consent form has been completed and returned to us.

By signing this form, I understand that my participation in this activity is voluntary and I freely consent to be recorded and these recordings to be used by UCAS for the following purposes:

1. Content for marketing materials
Your personal data may be used in marketing materials in order to encourage, motivate, inform, or inspire people to consider pursuing educational or career opportunities.

2. Content for commercial materials
Your personal data may be used to advertise or promote UCAS’ products and services.

3. Content for news media outlets
Your personal data may be used by us and news media outlets for articles, editorials, and other content to inform the public about educational or career experiences or opportunities.

4. Uses required or permitted by law
Your personal data may be shared where we are required or permitted to do so by law.

UCAS is committed to processing personal information in accordance with the General Data Protection Regulation and the Data Protection Act 2018 and further details, including your rights, can be found in our privacy policy available at: A specific privacy policy on the use of images is below.

I agree that I wish to participate and I hereby consent to the capture of my image in photographs/footage and grant to UCAS the non-exclusive right to use and reproduce my image (including by copying, adapting and by issue and communication to the public) for marketing purposes, online and/or via other digital/analogue methods; and to authorise such use and reproduction as may be deemed necessary.

To the extent that any audio/video recording in which I feature captures a qualifying performance under the Copyright, Designs and Patents Act 1988, I hereby authorise UCAS to reproduce, distribute and make available the recording of my performance(s) to the general public.

I confirm that I am aged 16 or older.

Withdrawing your consent

You may withdraw your consent at any time prior to the photography, filing or recording. Subsequently, if you wish to withdraw your consent for the use of your image please contact us at: datagovernance@ucas.ac.uk. Where a valid request is received, we will make every effort to remove or take down your image from UCAS communication channels.

UCAS’ privacy policy for the use of images, including video footage

This privacy policy tells you about the personal information we collect from you, how we use it, and what your rights are in relation to the capture of your image. Please note that the term image is used to cover both photographs and video footage in this policy.

As noted in the consent form we use photographs, videos and related content for the following purposes:

  1. Content for marketing materials
  2. Content for commercial materials
  3. Content for news media outlets
  4. Uses required or permitted by law

Our lawful basis for processing your information is consent, which is obtained before capturing your image. We will hold your image in the form of photographs or video footage together with the associated information you have provided in the consent form.

Your image may be published on any of the following channels, or other areas where appropriate:

  • press and media (including newspapers, magazines, websites and social media)
  • on our websites
  • on our social media feeds including Twitter, Facebook, LinkedIn, YouTube and Instagram
  • in our printed publications
  • in printed publicity literature such as leaflets, posters or other display materials

Although your image will initially be processed by UCAS within the UK, websites and other online media can be seen throughout the world, and not just in the United Kingdom, where UK law applies.

Your image will be used in relevant campaigns and your consent form will be retrained for the period in which the image is in use. The duration of a campaign or use of your image may vary dependent on its content; however, it is likely that footage will be retained for no longer than 7 years, expect where it is retained for archival purposes.

Data protection rights

Under the Data Protection Act 2018 you have a range of rights over your personal data, those relevant to the use of images include:

  • The right to be informed how your image will be used (provided via this policy and consent form)
  • The right to receive a copy of your data upon request
  • The right to have your data deleted
  • The right to withdraw your consent
  • The right to have inaccurate data corrected (this is unlikely to apply, unless the details listed in your consent form change)
  • The right to object to the use of your data or limit how we use it (this is likely to only apply where you wish to withdraw consent)

If you would like to apply these rights, or have questions about UCAS’ use of personal information, you can write to our Data Protection Officer via the details noted above.

If you are unhappy with the way UCAS had handled your information you have a right of complaint to the Information Commissioner’s Office. Details of how to make a complaint can be found on their website, but they would normally expect the complaint to be raised with UCAS first.
